NEW YORK — Datavant, a leading healthcare data collaboration platform, has launched HealthForms, a digital solution designed to simplify the completion of forms requiring provider signatures, including Family and Medical Leave Act (FMLA) and disability paperwork.

The new platform aims to reduce administrative workloads for providers while giving patients a faster, more seamless experience. By digitizing the process, HealthForms eliminates the need for staff to print, fax, and manually follow up on forms, cutting turnaround times by an estimated 50 percent.

“For too long, providers have had to divert their clinical teams away from patient care to handle tedious paperwork,” said Stephen Mallinak, GM and President of Datavant’s Provider division. “HealthForms reduces that burden by radically simplifying this process. By delivering a digital-first, secure, and patient-friendly solution, we’re helping providers improve operational efficiency and patient satisfaction at the same time.”

Traditionally, completing FMLA, disability, and similar patient-requested forms has been a manual and time-consuming task, requiring coordination with multiple third parties and often going unreimbursed. Datavant’s new tool automates data collection, payment processing, and delivery logistics, allowing clinical staff to focus solely on reviewing and signing forms.

HealthForms builds on the technology behind Datavant’s patient request tool, which has achieved a 95 percent patient satisfaction rate. The platform is available nationwide and supports a wide range of patient-requested forms, with the company positioning it as a significant step toward improving both provider efficiency and patient care.
