Commonwealth Care Alliance Senior Care Options Plan Earns 4-Star Quality Rating from CMS

BOSTON — Commonwealth Care Alliance (CCA), a mission-driven healthcare organization focused on serving individuals with complex medical and social needs, has earned a 4 out of 5-star rating from the Centers for Medicare & Medicaid Services (CMS) for its Massachusetts Senior Care Options (SCO) (HMO D-SNP) plan as part of the 2026 Star Quality Ratings. The plan provides specialized care for individuals aged 65 and older who qualify for both Medicare and Medicaid.

The CCA SCO plan received top marks in several areas, including Special Needs Plan (SNP) Care Management, Transitions of Care, and Reducing the Risk of Falling. The organization also achieved high ratings for behavioral health management through early detection screenings and specialized programs, along with improved pharmacy performance in measures such as statin use for diabetes, medication adherence for hypertension, and cholesterol management.

More than 45 percent of CCA SCO members live with four or more chronic conditions, and over 26 percent have a major physical or behavioral health disability. Nearly 59 percent are nursing home certifiable but remain safely at home with CCA’s coordinated care and support.

CMS’ star rating system helps beneficiaries compare Medicare Advantage and prescription drug plans based on quality and performance. Plans are rated on a scale of 1 to 5 stars, with higher ratings reflecting better care and service.

Earlier this year, CCA joined the CareSource family of companies, a nonprofit managed care organization serving over two million members across 14 states. In addition to CCA’s recognition, CareSource’s Dual Eligible Special Needs Plan (D-SNP) in Ohio received a 4.5-star rating from CMS.
